Request a Transcript
To request a transcript from the School for Social Work, download and print the SSW transcript request form (PDF). Fill out the form completely, sign it and send it to the Office of Academic Support Services. You may also fax or email it as an attachment with all of the information requested on the form. Note that federal law always requires your signature when requesting a transcript. We do not fax transcripts to third parties.
Please note that field placement information is included in the transcript under Winter Terms or Field Placement Term.

While there is no fee for processing, we do require that you allow 10 to 15 business days if you are requesting more than five transcripts.
From mid-May through early October, requestors should allow 5 to 10 business days for processing orders for one to four transcripts, in addition to mailing transport time. To expedite processing, you may send a pre-paid overnight or priority envelope (UPS or USPS only; we do not accept envelopes from Fed Ex). Note that some overnight services require delivery to a street address only (no P.O. boxes).
Official copies of transcripts may be withheld based on outstanding financial or other obligations to the college.
DEGREE AND ENROLLMENT VERIFICATION
Smith College has authorized the National Student Clearinghouse to provide degree and enrollment verification.

If you would like the Office of Academic Support Services to produce an enrollment verification on letterhead, please fax or email a written request to (413) 585-7994 or firstname.lastname@example.org and allow 5 to 10 business days for processing.
Your request should include the following: name of student, term to be certified, name and address or fax number of recipient, name, phone number and email address of person making the request and any other relevant information that we should include in our letter.
