Faculty Request for Office Space

Please fill in the information requested below. Offices are often shared, so an indication of whether you will need hours in the morning, afternoon, or both would be appreciated. Hours are 8:00a.m. to 12:00 noon for mornings, and 12:00 noon to 6:00 p.m. for afternoons. Please note that office buildings open later in the morning on weekends.

Office space is limited. If you need an office, we willl certainly provide one. However, if you don't anticipate a need for an office, please do not request one. Meetings can be held in the library if needed. If this form is not submitted, office space cannot be guaranteed.

Form deadline:  First week of April

Note: There will be a $15.00 deposit required for each key which will be held and returned at the end of the summer.
