PH.D. ADMISSION APPLICATION FORMS

Below are the Ph.D. Admission Application Forms required for application.

leafSmith is going green! We are no longer offering paper applications. Please consider the environment before printing your forms.

For your convenience, forms are available in two formats: online and PDF forms which can be filled out either electronically or by hand.

Deadline:
The School begins to process applications on September 1 for enrollment into the next academic year, which starts in June. While the deadline for admission is February 28, preference is given to applications filed before February 1: those received after that date will be considered to fill remaining vacancies. Special requests for filing an application after February 28 should be directed to the Office of Admission.

2. Official Transcripts

Please arrange promptly for official copies of both your undergraduate and graduate transcripts bearing signature and seal to be sent to the School from each educational institution attended. Applications cannot be reviewed without transcripts on file.

3. Five Letters of Recommendation

We prefer that you fill out the online Reference Writer information section of the application (Part 2 above) and provide email addresses for your reference writers. This will enable us to contact your letter writers electronically to request your letters in a timely fashion. Should you prefer to contact your reference writers directly, please include the form below.

4. Career Statement and Learning Goals

Comment briefly about the basis of your choice of social work for a professional career, and on how your family of origin may have influenced that choice. Describe the salient features of your professional experiences in practice, supervision, administration, teaching, research and community service. On the basis of what you have learned about your work, and about yourself; specify your aims for a course of advanced study, your research interests, and the professional questions or problems on which you would like to work further.

Describe the major educational opportunities you have had since graduation (such as supervision, psychiatric consultations, attendance at seminars, institutes) and evaluate their benefits to you.

List papers presented at professional meetings, publications, and other reports showing evidence of your capacity to undertake advanced study. If possibly, append reprints or copies.

If an applicant enters the program, this Career Statement becomes a part of the student’s confidential file which is available to all faculty. The information in this file may be shared with authorized members of the faculty and staff of the School.

5. Miller Analogy Test Scores

Please arrange to take the Miller Analogy Test (MAT) as soon as possible.
Click here for a list of official MAT Testing Centers. The Smith College School for Social Work MAT code is 1227.

Please submit Miller Analogy Test (MAT) scores in support of your application no later than the application deadline.

6. Example of Applicant's Clinical Work

Please provide an example of your clinical work.

Include a brief history of the presenting problem, and present a psychosocial assessment of the case including a clinical diagnosis and a tentative treatment plan. Describe an interview or parts of an interview which demonstrates the beginning direction of treatment and discuss the noteworthy features of this content.

Material should not exceed 10 double spaced pages and should be well disguised. If you have not been in direct practice for some time, please choose a suitable case from whatever sources are available. Describe the source and address the issues as indicated above.

The case submitted should include a detailed face sheet, disguised identifying data and process recordings.
