Certification of Completion Of Requirements
All seniors are required to demonstrate that they have or will complete the requirements of their designated major(s)/minor along with all other requirements for the degree.
A Certification of Completion of Requirements form is sent to all second-semester seniors at the beginning of their final semester. The form should be completed in consultation with the student's major(s)/minor adviser(s), signed by the adviser, and submitted to the Office of the Registrar by the end of the first two weeks of that semester.
The certification form is a requirement for graduation. If a student needs another copy of the form, she should contact the registrar's office. If a student makes a change to her current registration which involves courses required for the major(s)/minor, recertification is necessary.
Seniors who have a double major must complete a separate certification form for each major. A similar form is required for a minor.
