Certification of Completion of Requirements

 

All seniors are required to demonstrate that they have or will complete the requirements of their designated major(s) along with all other requirements for the degree. A Certification of Completion of Requirements form is sent to all second-semester seniors at the beginning of their final semester.  The form should be completed in consultation with the major adviser, signed by the adviser, and submitted to the registrar's office by the end of the first two weeks of that semester.   

The certification form is a requirement for graduation.   If a student needs another copy of the form, she should contact the registrar's office.  If a student makes a change to her current registration which involves courses required for the major, recertification is necessary.

Seniors who have a double major must complete a separate certification form for each major.  A similar form is required for a minor.
