Club Sports Recognition Form Addendum
Club sports teams are also required to complete the Club Sports Recognition Form Addendum, and submit it online each year. Club sports are accountable to both the Department of Athletics and the OSE and must keep both offices informed of changes in the leadership. Once you fill out this form, you will be prompted to fill out the addendum.
Please print this page before sending it to the OSE so that you have a copy for your records.
