DOCUMENTATION OF ATTENTION DEFICIT DISORDER

Students, faculty, or staff diagnosed with Attention Deficit Disorder (ADD/ADHD) who are requesting accommodations or services through the Office of Disability Services are required to provide appropriate documentation of their disability. The guidelines describe the necessary components of acceptable documentation. You are encouraged to share these guidelines with your physician, psychiatrist, or psychologist. Additional questions about documentation or accommodations may be directed to the Director of Disability Services.

In order for documentation to be complete, each of the following items must be included in the documentation report:

	Diagnosis and corresponding DSM-IV code with the date of the diagnosis.

	Date and nature of the last contact with person.

	Specific information regarding the instruments and assessment procedures used in the evaluation and results of the assessment.

	A description of the current manifestations of symptoms that meet the criteria for diagnosis.

	A statement of how the disability and/or related medication and treatments interfere with or limit any facet of major life activity including current employment or participation in courses, programs, services, or other activities of the college.

	Information regarding any current medication, including dosage and frequency.

	The diagnostician’s name, title, address, phone number, and signature.
