- A $100.00 NON-REFUNDABLE DEPOSIT must accompany this registration form. It will be applied to the total cost. Confirmation of enrollment will be sent via email only.
- Balance due must be paid by July 1.
- There will be NO REFUNDS after July 1.
- Final instructions will be emailed two weeks before the program begins.
- Space is limited. Processing will be according to postmark.
PLEASE NOTE: STUDENTS UNDER AGE 10 MUST COMMUTE TO THE PROGRAM AND BE DROPPED OFF AND PICKED UP EVERY MORNING AND AFTERNOON!

All participants must print, complete, sign, and return four medical documents: Medical Information Form, Prescription Medication Policy, the Medical Release Waiver Form, and the Meningitis Vaccination Waiver. Smith College requires either proof of vaccination or a signed waiver form. Signed forms must be returned by July 1 to:
