Please attach a list of e-reserve documents. For each item, include:

Complete bibliographic citation
Copyright statement (e.g., © 1987 by Oxford University Press)
If material is being reused or exceeds fair use limits, please indicate who provided copyright permission for use of the item (Copyright Clearance Center, publisher, author, etc.).

Provide clean, crisp, one-sided photocopies of all hardcopy items and electronic copies of all PDF, HTML, MS Office and other electronic files.

If you know an item is available full-text (e.g., J-STOR, Lexis/Nexis, online subscription), you do not need to provide a print or electronic copy. On your list, indicate where the item is available and we will upload the item to Moodle.
