Video Booking & Borrowing Form

Smith faculty, students and staff can use this form to:

  • Book Smith or Five College videos for specific screening dates. Note: Most DVDs & videos can now be requested through the Five College Library Catalog.
  • Also use this form to request Smith films in storage.

Please check the Five College Library Catalog for availability and borrow Smith videos in person from the Neilson or Josten Library circulation desks. See Find DVDs & Videos for more information.

Guidelines
Deadline: 5 working days in advance
Limit: 3 requests per week
Loan Period: one week with no renewals
Pick up and return: We will send an email when the item arrives  - it will be held for 3 days at the Neilson Circulation Desk.
