Student Insurance & Liability Waivers
Some employers require interns to obtain liability waivers from their schools. Students who are asked to provide a waiver must go through the Five College Risk Management Office. Scroll to the bottom for Smith College instructions.
Supervisor Confirmation Form
You are required to submit a completed Supervisor Confirmation Form* AND an attached letter on official letterhead or email from your host organization confirming the offer of an internship and describing your internship duties and how you will be supervised. Your supervisor may print out the form (PDF) or complete it online by clicking the link above.
Letter for Supervisor
Give the Supervisor Letter to your supervisor when you first arrive at your workplace. This letter contains important information related to your Praxis funding. Your supervisor will use this form to comment on your job performance and confirm that you worked a minimum of 220 hours. This letter will also notify your supervisor of his or her need to help you fill out a learning contract at the end of your internship. This document will be given to you once your completed application has been approved.

Learning Contract
With your internship supervisor's help, fill out the learning contract at the beginning of your internship. By going over a contract with your supervisor you will be able to understand what will be expected of you, and you will be able to communicate your learning goals to your supervisor. This contract is for you to keep and to refer to as needed. You do not send a copy of the learning contract to the Lazarus Center.
This document will be given to you once your completed application has been approved.

Over the summer Lazarus Center staff will be in touch via e-mail with you and your supervisor. This is a chance to hear your thoughts and your supervisor's feedback on how your internship is progressing.
- Expect to do clerical work in addition to career–related projects
All interns do routine office work. All permanent staff do as well. You will be valued by your co-workers and supervisor if you pitch in cheerfully where needed. Your time as an intern should be divided between clerical work and more substantive assignments that will enable you to work toward your learning goals, with the emphasis and balance of your time on the latter.
- Call the Lazarus Center if there are problems
If you encounter difficulties with a supervisor or co-workers, an unexpected change in assignments or an internship that varies significantly from your expectations, it is important for you to speak with a Lazarus Center advisor as soon as this situations arise. We can help by working with you to find a solution to the problem.
- Notify the Lazarus Center if you cannot complete your 220 work hours
If you are forced to leave your internship before you have completed 220 work hours, you must contact the Praxis Data Assistant, Kimberly Lebron, before you leave. Students who are given Praxis stipends are expected to fulfill their commitment of 220 work hours. If unforeseeable and unavoidable circumstances dictate that must leave, you will be obligated to reimburse Smith College a pro-rated portion of your Praxis stipend. The amount you owe will be placed on your next tuition bill.
