Coordination Record is a secure, HIPAA-compliant cloud-
based platform that connects dynamic care teams and drives
action. It is accessible via web browser on desktop, laptop,
tablet, or smartphone, and thru SMART can be embedded in the
EMR. Patients with complex needs require thoughtfully designed care plans and high-quality execution. ACT.md is purpose-built to support complex team interactions and workflows that involve patients, families, and a broad array of care providers each contributing aspects of a complex care plan. Care teams need to work together to manage patients’ medical, behavioral, functional and social needs. ACT.md exists to make this possible.

City Year works to bridge the gap in high-poverty communities between the support that students actually need and what their schools are designed and resourced to provide. In doing so, we’re helping to increase graduation rates across the country and changing lives of the students we serve. At City Year’s 27 locations across the United States and two international affiliates, highly trained young adults, our AmeriCorps Members, work full-time in schools for 11 months providing individualized supports to at-risk students. We partner with schools and districts to identify students who are at risk of not graduating on time with their peers, and provide them with extra academic, emotional and social support an skills that they need to be successful for college and a career.

iD Tech Camps

iD Tech Camps was designed for aspiring programmers, app developers, video game designers, robotics engineers, 3D modelers and animators, filmmakers, photographers, and more. At these weeklong, day & overnight summer camps, courses are led by tech-savvy, adult-only staff in a fun, experiential environment. With personalized instruction for all skill levels, and small class sizes of just 8 students per instructor, students will be creating and innovating in no time!
