Pre-Departure Information Form – 2011
(for non-graduating students)

In order for this office to issue appropriate travel papers and keep accurate records, all international students must complete the section(s) which apply to them and return it to the Office for International Students and Scholars no later than Friday, May 6, 2011. Please read carefully and complete as much as possible. You need only to complete the section, which applies to you.

If you are leaving the U.S. this summer please have your I-20 signed by Dean Tamzarian (Wright Hall 124) or Ashavan Doyon (Clark Hall 303). You can drop off your I-20 in our office anytime between now and May 6.
