FOR CURRENT STUDENTS
Pre-Departure Information Form – 2011
(Seniors, AMS, Graduate Students)

USCIS has implemented new regulations which require colleges to report address and employer information for everyone doing OPT.   Students on OPT will be required to report their initial OPT Employer address and personal address to this office as soon as they know. Any changes in address and/or employer during the OPT period should also be reported to our office within 10 days of the change.  In turn we will update your information in SEVIS.

Taxes: Our office is also going to send you the tax forms and the Cintax link and password the year after you graduate from Smith so that you can file your taxes. This request is for everyone including students on OPT, Graduate school or at home. If you want any monies which were held from you to be sent by IRS, please give us your address and/or permanent Email after graduation. Tax forms will be sent in February/March of 2012.

B: GOING TO GRADUATE SCHOOL IN THE UNITED STATES
In order to transfer from Smith College you need to complete the Transfer Form from your new school. Please ask the Office for International Students & Scholars to send you the form and then bring it to our office. We will complete our part and give you a Transfer Release Date. Then we will mail or fax the form to your new school so that they can issue an I-20.

C. OPT AFTER SMITH
If you currently do not have a job yet, please give us a summer address.  Remember, you are now required to report the name and address of your new employer to us as soon as you find a job. You have 90 days from the start date on your OPT card to find a paying job and/or find a volunteer position and report it to our office. Otherwise your OPT will be automatically cancelled.
