Health & Wellness Forms

Health Care Plan

Smith offers two health plan options through Harvard Pilgrim Health Plan. You may elect either a Point of Service Plan (POS) or an HMO. Health insurance premiums are deducted from your salary on a pre-tax basis. If your date of employment falls on the first day of a month, your eligibility begins on that day. Otherwise, your eligibility begins on the first of the month following your date of employment. There are no pre-existing condition restrictions. The college contributes 92.4% of the premium for the HMO individual plan and 75% of the premium for the HMO Two-Person and Family plans. The college’s contribution towards the POS plans is the same dollar amount that it contributes to the HMO plan. In addition, the college provides a health care subsidy (from $401 to $1603 per year) for employees who carry double or family coverage and whose family income is less than $43,890.

Summary Plan Description (SPD)

Dental Plan

Smith offers dental insurance through Delta Dental. This plan covers 100% of preventive dental expenses and 50% of basic restorative and major restorative dental expenses. The maximum benefit is $1000 per family member per calendar year. The college contributes 89% of the cost of coverage for individual plans and 60% of the cost for double and family plans.

Life Insurance

The college provides you with Group Term Life insurance through The Hartford Life Insurance Co. The coverage is equal to your annual salary rounded to the next highest $1,000. You have the option of purchasing additional coverage equal to one, two or three times your annual salary. You may also purchase life insurance for your children and your spouse or domestic partner.  For a Supplemental or Dependent life insurance application, please contact Human Resources at (413) 585-2270.

Long-Term Disability Insurance

The college provides you with basic long-term disability insurance through The Hartford. If a physician determines that you are physically or mentally incapable of doing your job and your claim is accepted by The Hartford, you would continue to collect 50% of your salary. You may also purchase additional supplemental insurance to cover 66 2/3% of your salary. If approved, income benefits become payable 180 days from your last day worked.
