Latex Orders

Condoms, dams and gloves are available at clinic costs to the houses through Smith Health Services (prices are far below retail costs). Orders for each house should be placed by the House Health Peer Rep via online using the Latex Order form below. Order forms should be filled out and sent via attachment to latex@smith.edu. Health Services will provide a small budget based per capita. If the balance allocated by Health Services is exceeded, the house should cover the additional cost of the order with its own funds. Private cash/check orders may be arranged through the Health Peer. Orders will be filled the quickest if submitted by Wednesday morning.

*Note: Please be sure to include a statement of Health Services current house balance, payment, or have your treasurer transfer payment electronically (see form).
