Current and potential America Reads agencies & schools should download the following PDF forms. Keep the forms on file that provide information on the program
and complete, sign and return those forms necessary to contract eligible students to work at the agency. Agencies should update their job descriptions
if they change and new agencies must submit the IRS Form 501-C-3. Forms necessary to complete a yearly America Reads file include:
Work-Study Program Information for Off-Campus Agencies (retain for your records)
Agency/School Information Sheet (to be completed and returned to the College)
America Reads/Counts Tutoring Position (job description(s)to be completed and returned to the College)
School/Agency Procedures for America Reads/Counts Tutors (retain)
- Instructions for completing the Banner Application (retain)
- Banner Application (complete and return to the College)
Agreement for Participation in the Federal Work-Study Program - America Reads/America Counts
(to be signed by the authorized organization representative and returned to the College)
America Reads/America Counts Tutor Contract (complete and sign one for each student hired - the college representative completes the calculations portion)
