To request a transcript from the School for Social Work, download and print the form from the link below. Fill out the form completely, sign it, and send it to the Office of Academic Support Services. There is a $3.00 charge per copy. There is no charge for currently enrolled students. Please allow five (5) business days for processing.
Please note that Field Placement information is included in the transcript under Winter Terms or Field Placement Term.

Your request should include the following: name of student, term to be certified, name and address or fax number of recipient, name, phone number and email address of person making the request (in case we have questions), and any other relevant information that we should include in our letter.
