Last year we introduced a web-based version of the Faculty Record Sheet. The new form is intended to facilitate form-completion, record-keeping, and data collection. Beginning this year all faculty will use the new online form to submit their 2011-12 Faculty Record Sheets and updated CVs. The completed form along with an updated CV is due September 15, 2012.
