FACULTY RECORD SHEET

Beginning with 2010-11 Faculty Record Sheets, faculty are invited to submit their FRSs and CVs through a newly-developed online form. Alternatively, you may complete the Faculty Record Sheet form using the Microsoft Word document version and submit it by e-mail attachment (along with an updated CV), or complete a paper version of the Faculty Record Sheet form, and mail it by campus mail (along with an updated CV).

It is the goal of our office for all faculty to be using the new online form for 2011-12 Faculty Record Sheets.

Complete the form using the Microsoft Word document.

 

Instructions for completing and submitting this form:

Save the document to a desired location on your computer.

Complete the document, using the "tab" and arrow keys to navigate the form.

Send the form and a copy of your current CV as e-mail attachments to hspizz@smith.edu.
