ODS Online: Forms - Contracts Request Form
For any event that you are hiring a performer, speaker or artist, paid or unpaid, you must fill in the form below and submit it to sacr@smith.edu. From your request, a contract will be generated and mailed out to the talent. Once the signed contract is returned, you will be contacted to pick it up from the Campus Center Office, room 106 or the Office of Residence Life in Clark Hall and submit it to the Student Bank for payment.
