Medical Clearance Procedures
Intercollegiate Athletes: (Scroll down for Club Sports)
All first year and transferring student-athletes and any other athlete participating in college for the first time will need to have a sports physical within 6 months of their first practice date. See chart below to check if your physical exam will be current for your first practice.
| Volleyball, Soccer, Field Hockey, Tennis and Cross Country | after February 25, 2008 |
| Riding, Crew, Lacrosse, Softball, Swimming and Diving, and Squash | after March 1, 2008 |
| Skiing, Track and Field, and Basketball | after April 15, 2008 |

Health Services Physical Form 2008
Your physician must indicate that you are cleared to participate in athletics on the form.
Returning athletes and newcomers need to have an updated medical history form, updated insurance information, waivers and an updated measurement of height, weight and blood pressure before beginning participation each year. These measurements will be conducted by the athletic trainers by appointment only. When you fill out your online paperwork, you will be contacted about coming in for these measurements.

ImPact Testing: Athletes particpating in volleyball, soccer, field hockey, equestrian, lacrosse, softball, diving and skiing must complete this test before beginning practices. This baseline test is only done once in your career. If you have already completed it once, you SHOULD NOT take it again.

Club Sports:
Before stepping foot on a pitch, court,
or field, be sure to have the appropriate forms and procedures completed
so that you are ready to play at the first practice. All club athletes
must print and complete Form 1 and Form 2. The completed forms must
be turned in to the Athletic Training Room at least 48 hours prior to
a student-athlete participating in any athletic activity (i.e. practice
or competition).
